Children's Programs Registration Form

There are two ways to sign up for our Children's Programs:

1. CLICK HERE, and print out the registration page.  Then, either drop it off at the office or fax it in.

OR

2. Fill out the web form below, click submit, and the information will automatically be sent to the church for registration.  You will then receive an email confirmation from the church! 

3. PLEASE COMPLETE A REGISTRATION FOR EACH CHILD!
